1. Completed Attorney Questionnaire signed by a licensed attorney;
2. Police Report;
3. Medical records for the initial date of treatment after the incident;
4. Medical records indicating the injury received; and
5. Complaint, if filed.

Medical Malpractice

1. Completed Attorney Questionnaire signed by a licensed attorney;
2. Medical Expert’s Report or doctor’s certification letter;
3. Medical records for the initial date of treatment after claimed negligent act;
4. Medical records indicating the injury arising from claimed negligent act;
5. Complaint, if filed.

Worker’s Compensation

1. Completed Attorney Questionnaire signed by a licensed attorney;
2. Workers Compensation Incident report;
3. Medical records for the initial date of treatment after the incident;
4. Medical records indicating the assessed permanency rating; and
5. Medical Examination Report (ex.: IME, QME, AME), if available.

Settled Cases

1. Completed Attorney Questionnaire signed by a licensed attorney; and
2. Copy of the fully executed final Settlement Agreement that includes the signature of both the plaintiff(s) and the defendant(s) in the case.
